Role Overview: Field Care Assessor
As a Field Care Assessor with Home Support Matters, you'll play a vital role in supporting our care and support workers to deliver person-centred, safe, and effective care across our community. You'll act as the crucial link between customers, their families, our care teams, and external professionals, ensuring a seamless and high-quality service.

Hours:
37.5 hours per week, Monday to Friday, 9am to 5pm

Participation in an on-call rota (including every other weekend) with enhanced pay of £15.97/hour

Flexibility to work outside of core hours to meet service demands

Full-time, part-time, or job share opportunities available!
Key Responsibilities:
Conduct detailed care assessments and develop personalised support plans

Ensure accurate medication management and comprehensive care documentation

Perform regular audits to maintain high standards and compliance

Collaborate with multidisciplinary professionals including social workers, GPs, and family members

Provide coaching, support, and supervision to care/support workers in the field

Promote independence, choice, and dignity in every aspect of care delivery

Respond promptly to any safeguarding concerns or incidents

You'll cover areas including Beccles, Lowestoft, and Great Yarmouth, based from our Beccles head office. Mileage is reimbursed at 42p per mile.

Who We're Looking For:
We're seeking candidates who bring:

Experience in domiciliary or community-based care

A genuine passion for delivering high-quality care

Excellent communication and interpersonal skills

Confidence using Microsoft Office and digital care systems

Strong organisational skills and attention to detail

A full UK driving licence and access to a vehicle

NVQ Level 3 in Health & Social Care (or willingness to work towards it)

Supervisory or team leadership experience (desirable but not essential)
