Description
Completing and implementing clients' care plans including risk assessments.
Providing support with personal care.
Supporting and administering medication.
Supporting with physical health conditions, emphasising kindness and independence.
Ensuring precise documentation and effective communication.
Making referrals to other healthcare professionals for further assessment or intervention.
Completing carer's supervisions, client reviews and amending changes to care plans.
Carrying out feedback forms, spot checks, supervisions, and appraisals on care staff, identifying any areas of concern and applying corrective or preventative measures.
Covering shifts during the on-call service, and to build long lasting relationships.
