37 hours per week

Fixed term contract until 31 January 2019

It’s an exciting time to join us as we embark on a new approach to care. An approach that seeks to embed our vision to support people to be independent, resilient and well.

As part of our promoting independence agenda we’ve introduced new roles that will support and influence discharge pathways. Based within the acute hospitals these roles will work closely with patients, families and providers along with health and social care staff to streamline existing processes to reduce delayed transfers of care.

As a Trusted Assessment Facilitator you’ll build mutually beneficial and trusting relationships between hospital discharge teams and homecare, nursing and residential care providers to ensure support is in place to facilitate timely and safe discharge.

We’re looking for people who are innovative with a confidence and focus to really connect services and the people within.  You’ll work closely with care home managers and care providers as well as discharge teams to determine the level of assessment needed, producing outcomes plans that reflect a person centred approach from hospital through to community settings.

You’ll already have an NVQ level 3 in health and social care or equivalent and understand local and national policies relating to the care sector. You’ll have experience of working in either a provider, hospital or a care home environment and will have been involved in the discharge process.
