Norfolk First Support are working in partnership with the Clinical Commissioning Groups to develop a new Supported Care model, which is about making adjustments to the way services are organised so that we reach certain patients before they reach crisis point. The programme aims to keep people at home and help to maintain independence as long as possible. It is part of an on-going process to establish local joined up community provision, connecting health and social care with the voluntary sector and wider community.

Working across health and social care and in partnership with patients and their families and carers, the voluntary sector and the wider community.

We are looking for the following people to join our Norfolk First Support team on this new exciting venture. We do require a commitment that you will work an alternate weekends.

Reablement Practitioner – Supported Care

28.5 hours Wymondham/ North Walsham

Coordinators – Supported Care 30.5 hours North and 26.5 Hours South

Deal with new referrals from health professionals deploy staff and line management responsibilities

Reablement Support Workers – Supported Care

South -Main base will be Wymondham, but also looking for staff to work in Thetford, Dereham, Diss and Poringland areas

North – Main base North Walsham, but also looking for staff to work in Fakenham and Broadland areas

Night assessors – Supported Care

All posts will be based either at Wymondham or North Walsham

Hours range from 21 to 37 per week, with various shift patterns available Salary ranges from £9.36 – £12.93 per hour depending on role
