As a Reablement Practitioner you will undertake assessments to establish reablement goals, determine levels of functional independence, and reach desired outcomes with service users.  You will also line manage a team of Reablement Support Workers who work directly with people to assess and support service users during their period of reablement.  The vacancy covers all areas across South Norfolk.

 

Norfolk First Support delivers services to adults in Norfolk in their own homes for up to 6 weeks, assisting them to regain skills they may have lost following an illness or hospital admission. The aim is to enable them to regain their independence as much as possible while also reviewing their ongoing care needs.

 

Norfolk First Support is part of the wider Norfolk First Response Service which consists of Swift Response, which supports people with unplanned care needs, Supported Care and Accommodation-Based Reablement units.
