The Role

We are looking for individuals to provide aftercare services to any person assessed by the Norfolk and Suffolk Foundation Trust (NSFT) mental health professional whilst in hospital, who would benefit from, and agrees to, support from Norfolk and Waveney Mind.

The roles will improve the post discharge experience for this group of people by providing 1:1 short intervention intensive support, for no less than 2 weeks and more than 12 weeks. You would be working to link the individuals within the wider community and mental health services so they can navigate their own recovery pathway beyond the remit of the NSFT services.

Excellent organisational skills are essential to manage your own case load and meet the specific contact requirements.  The position will suit anyone who has a passion for providing high quality care and helping others experiencing difficulties in their life.

The successful candidates must also have a driving licence and their own car. The role itself is Monday to Friday and does not require shifts or weekend working.  Travelling time between visits is included within working hours and we currently pay a rate of 0.45p per mile for travelling expenses.

Experience

We require individuals to have a minimum of 2 years previous experience in the same or similar field and understand mental health. Experience of working with people with complex needs and those difficult to engage is essential.
