£22,183 to £24,491 per annum, pro rata for part time

Norfolk County Council has a shared vision ‘“To enable young people with learning difficulties and/or disabilities, including mental ill health or autism, to live the lives they want by having the same access as their peers to opportunities for employment, housing, community inclusion and good health”. To achieve this Adult Social Services and Children’s Services have worked together to develop and enhance the current transition support through the Preparing for Adult Life Service, which provides a seamless and consistent preparing for adult life experience for young people and their families.

You will have the ability to work with young people from age 14 up to 19 years with complex needs, as well as their families. You will be able to demonstrate an ability to work with young people and their families in a person-centred way to promote young people to achieve their 4 PfAL outcomes of employment, being healthy, being part of the community and being independent. The role will involve writing strengths based PfAL Plans with young people and their families as part of a ‘PfAL Conversation’ and ensuring young people are connected to the right services to meet their needs.
