The role:

The Digital Team is looking for an experienced Digital Change Facilitator to work with the Digital Project Manager for Care Homes promoting the principles of the Enhanced Health in Care Homes (EHCH) model care element of better technology.

As a Digital Change Facilitator you will be instrumental in establishing a new culture of positive change through technology, providing support, training, digital expertise, enthusiasm, and solutions.  We’re looking for someone with experience in a similar role, with a good knowledge of Care Homes across Norfolk and Waveney and an understanding of the digital challenges they face.

Main duties:

  • Providing support to staff in Care Homes and the CCGs in the adoption of new technology and ways of working.
  • Actively supporting facilitated discussions in relation to the scoping, planning and delivery of digital solutions within Norfolk and Waveney.
  • The design and delivery of change programmes to enable the achievement of agreed goals and expected benefits, connecting with staff and patients to facilitate and prepare them for the transition to new ways of working.
  • Supporting the development of the Norfolk and Waveney Primary Care Digital First programme capability by increasing our ability to accommodate and adopt new and innovative digital solutions and opportunities.
  • Improving the adoption and usage of digital tools and solutions with staff, clinicians and patients and act as a digital champion for these products and initiatives.
  • Review and evaluation of identified digital solutions and initiatives
