Requirements:
- Have Level 3 Diploma in health and social care. Experience in care work minimum of 2 years.
- Fully flexible working patterns must be driver with own car, work both Am and PM which includes weekends, short notice periods to cover for emergencies when required.
- In return we offer set working pattern full support and training given.
Duties include:
- To be responsible to the Registered Manager for the day-to-day provisions. To keep area Manager up to date with information of any outcomes or issues that have been identified
- be aware of CQC standards and support Leaf to keep up to standards, health and safety will be included.
- Meeting Deadlines and completing tasks in line with the requirements asked of you.
- Carrying out supervisions of your staff
- Ensuring your team is completing their training and updating files accordingly
- Carrying out personal development plans with staff.
- Senior level training including risk assessments, medication, end of life, dementia, Train the trainer in moving and handling.
Reporting and dealing with:
- Social Services
- Occupational Therapists
- District Nurses
- Pharmacies
- GPs
- Sometimes maybe required to attend welfare meetings such as care plan reviews.
- Hospital discharges to ensure service users come home safely and care plans/risk assessments are up to date.
Rapid response duties:
- To cover for staff sickness across the board.
- Every other weekend rapid response if not on round if required.
- complete overdue and due tasks be out in the community supporting staff,
- care plan reviews – ensuring these match with care tasks and service user outcomes.
- Management of events to be dealt with and organisation of your work load to complete deadlines within your rapid duties.
- Spot checks completed with feedback any changes are correctly updated inline with service users’ requirements and health and safety.
- Ensuring tasks match the tasks carers are completing and ensuring the tasks meet the service users needs.
