We have an exciting opportunity for a suitably qualified and experienced individual, with a passion for improving outcomes for disabled children, to establish and lead a new outreach service for children with disabilities in Norfolk.
You will have a Social Work, Nursing or Occupational Therapy qualification, a knowledge of disability issues and experience of working with disabled children and their families.
You will lead a small team in providing direct care and support to young people and their families within their own homes, with the aim of preventing family breakdown and young people being accommodated.
The outreach service will work across the county with children with learning/physical disabilities and may include those with complex health needs and challenging behaviour.
You will be a positive role model, supporting and leading a small staff team which will include providing professional supervision, performance discussions, and planning and implementing training, including the induction of new staff.
You will have experience and knowledge of safeguarding procedures, will contribute to key planning and decision-making regarding children’s care, and will work in partnership with other professionals.
The position will involve being part of an on-call system and may include working evenings, weekends and bank holidays occasionally.
