Full time salary £24,982 to £27,041 per annum

We have an opportunity for a number of Assistant Practitioners to join us in a central team, providing support across the county.

 

You will be assisting Adult Social Care teams in supporting those people newly discharged from hospital. You will be assessing and reviewing individuals need for care and support, taking a strength-based approach and arranging services where required to make sure those needs are being met.

All our office-based roles are currently working from home as part of our Smarter Working plans and to support the health and safety of all our colleagues. We anticipate that working remotely will be a key feature of this role.  Our technology platform and equipment are first class, enabling you to connect and collaborate remotely. We ask that you have in place good Broadband connectivity.

 

There will be a future requirement once current restrictions are lifted to make face to face visits, so we are interested in hearing from candidates across the county.
