We are seeking enthusiastic Assistant Practitioners to join Eastern Community Care Team.  We work with older people, adults with a physical disability and their carers. We are a highly motivated, innovative team of Social Workers, Occupational Therapists and Assistant Practitioners.  We undertake Care Act assessments, support planning, reviews and provide information, advice and signposting.

 

You will have excellent communication, record-keeping and keyboard skills, along with experience of working with vulnerable people.  In addition, you should have a commitment to excellent customer service and have an understanding of the Care Act 2014.  You will have a strong commitment to multi-disciplinary working, promoting independence and be prepared to embrace the team’s practice of the 3 Conversation Model, which prevents, reduces and delays reliance upon formal social care.

Covid-19 vaccination requirements

The Health and Social Care Act 2008 (Regulated Activities) (Amendment) (Coronavirus) Regulations from 11 November 2021 means people working in or professionally visiting CQC-registered care homes will need to be fully COVID-19 vaccinated unless they have a medical exemption.  If the role involves visiting registered care homes, you will be legally required to show proof of full covid-19 vaccination or medical exemption and will need to provide evidence of this during the onboarding process
