Would you like to work alongside our well-trained team supporting the delivery of care and support in the community? Then this role is for you. 

Norfolk First Support delivers services to adults in Norfolk in their own homes for up to 6 weeks, assisting them to regain skills they may have lost following an illness or hospital admission. The aim is to enable them to regain their independence as much as possible while also reviewing any ongoing support that may be needed.

Norfolk First Support is part of the wider Norfolk First Response Service which includes Swift Response, Supported Care and Accommodation-Based Reablement units.

You will be part of a rapid response and assessor team for Norfolk First Support, specifically focussed on people who are discharged home from hospital and require support at home.  The role includes assessing and reviewing people’s daily living needs, developing a personalised plan to assist the person to regain the skills and confidence to remain independent in their home.

You will need to have experience of working with service users and be able to communicate clearly and appropriately with members of the public.  You will also need to be able to exercise judgement and initiative and be able to plan and prioritise your own work.

This is an exciting and challenging role.  You will work in a multi-disciplinary team and need knowledge of current policy, practice, legislation within Health and Social Care and enjoy working with members of the public and other professionals in Health and Social Care.

This is a full-time permanent post although part time applications are welcome.
