Salary:  £25,419 to £27,514 per annum

Hours:  37 hours per week

An exciting opportunity has arisen for an Assistant Practitioner to join our Central Norfolk Home First Hub and Norfolk and Norwich University hospital team.

The Home First Hub is an integrated team working closely with our Norfolk Community Health & Care (NCH&C) and NHS colleagues to ensure safe and timely hospital discharge for those in community and acute hospitals across the Central area. The Home First Hub works within the Discharge to Assess model and is a new and developing team committed to supporting people to return home, access reablement opportunities and plan their ongoing care and support needs once discharged.

The Home First Hub workers currently work from community hospital, Acute Hospitals and remotely. We are looking for a longer-term base to continue to develop this dynamic integrated team.

While this post is for the Home First Hub and offers a hybrid role of remote / office based this team is part of a large-scale transformation around the development of the Discharge to Assess model. This means that in line with service need and development your role may stay in the Home First Hub or move to assessing people in their places of residence in the community.

The role will also include onsite working in the Norfolk and Norwich University hospital to support with complex case reviews, Supporting Social Workers with Mental Capacity Assessments, Best Interest decisions and safeguarding. At times there may also be a need to support with assessing or reviewing a person needs while they are in hospital to support their discharge. This will include face to face visits with patients, discussions with families and the wide multi-Disciplinary team.

You’ll have experience in the Health or Social Care sector, have a good standard of education, and have excellent IT and organisational skills. You will be competent at working at pace in a responsive fast paced environment. A positive can-do attitude is essential alongside professional curiosity around information exchange and community resources. You will be capable of working independently as well as part of a team.

You will have good listening and analytical skills, be flexible and adaptable to new ways of working and be committed to supporting people to leave hospital safely. Some weekend working will be required.
