Salary:  £24,982 to £27,041 per annum

Hours:  37 hours per week

We have an exciting opportunity for enthusiastic Assistant Practitioners to be part of an existing county-wide team.

Our team has been set up as a strategy to support Adult Social Services to address the current levels of unallocated work and overdue reviews across our five Community care teams.

You will have some experience in the Health or Social Care sector, have a good standard of education and be committed to working in a person-centred way.

You will be centrally managed but based alongside Social Workers and Occupational Therapists within a Locality team working with older people, carers and working age adults with a physical disability. You will have a large geographical area to cover so a willingness to travel is essential. During COVID-19 restrictions most work is being carried out remotely, but you will have a nominal office base.

In this role you will be taking a strength-based approach to working with people, linking them with community resources in the first instance and in some instances carrying out Care Act Assessments. You will work closely with the Occupational Therapists in your team and receive training to provide low level equipment. You will need a ‘Can do’ attitude and be capable of working independently as well as part of a team.

You will have good listening skills, be flexible and adaptable to new ways of working and be committed to supporting people to maintain their independence in their own homes.
